Kevin Campbell lives in the dismissive shadow of his father, Frank. Long estranged, both are engineers with Kevin working for a large railroad, and Frank for a regional that is the lifeblood of their small town. Their friction is displayed at a local bar, where Frank’s work partner, Tom, enjoys his retirement party. It's a gathering that Kevin is dragged into by his wife and daughter. The following morning the regional announces it’s going under, wiping out most of the town’s employment and driving Tom to suicide in the locomotive he’s worked in with Frank for decades. Frank falls into an alcoholic depression and his grief crescendos into sabotaging the railroad’s trackage to avenge his best friend’s death. He targets a freight run, but instead derails a passenger train, killing nine people.
NTSB investigator Andy Mayland arrives and walks the wreckage with sheriff Hollis Peete, Frank’s mortal enemy. She finds Kevin at the site, who is too shell-shocked to speak and walks by without a word. Hollis spots Frank as he arrives in a maintenance train and is instinctively suspicious. Frank is in his own state of shock as everything around him was of his own doing. Frank deteriorates as guilt and the investigation closes in. Kevin, too, begins to suspect something, although he’s distracted by Andy while his marriage sags due to his ongoing warfare with his father. Kevin confides that all he wants his father’s approval, the one thing he has never gotten.
Now desperate, Frank joins Kevin on a scenic run in hopes to get his son’s help. It will be their only joyous moment together as Frank’s confession sparks a violent confrontation between the two. Andy has also learned the truth with Hollis moveing in to arrest Frank, igniting a final battle in a locomotive that becomes a runaway. Andy confronts Kevin about Frank, but word gets out about the brawl between Frank and Hollis in the runaway. Estrangement aside, Kevin decides to save his father. With Andy in tow, Kevin takes his own locomotives to chase after his father, eventually jamming them into the runaway. They escape onto Kevin’s locomotives after Frank defeats Hollis. Frank remains, ensuring the locomotives separate while Kevin, Andy, and Hollis escape to safety.
And in a final act, Frank tells his son “If you remember anything, remember you’ll always be the greatest thing I’ve ever done!”. Frank releases the coupler and rides to his fate, now at peace as he disappears into the darkness of an abandoned tunnel. Kevin, his need fulfilled, focuses on repairing his family and reflects on his father’s final moments. And he wonders if he were to look into his father’s eyes again one day, would he see himself.
